Application All loan applicants should complete the primary application form which can be viewed via the link immediately above. It is not necessary to complete additional Form Sets "A", " B", or "C", unless your company fits one of those three profiles described below. Applicants who are applying based on disadvantage should pick the profile that most closely matches their situation. Only one of the three additional Form Sets needs to be completed. Should be completed by applicants who are claiming "social disadvantage" as members of a designated group specified in 13 CFR 124.105(b), i.e., Black Americans, Hispanic Americans, Native Americans, Asian Pacific Americans, and Subcontinent Asian Americans.
Should be completed by individuals who are not members of a designated group, but are claiming "social disadvantage" on the basis of other factors cited in PPR# 2017.
Should be completed by individuals claiming "economic disadvantage (this must be substantiated by a completed personal financial statement and recent federal income tax forms.)
